Wudu' questions

[slm] What to do when: A- You're in the middle of making wudu' and you realize you left off an essential component. B- Same as A but this time you're in the middle of praying. Obviously one's wudu' is incomplete in both cases but do you go and do just the missed part or redo the whole wudu'? I always did the whole thing again but then i read somewhere that you can just do the missed part. Is anyone familiar with this? I also read that if you forgot a part of the wudu and remembered during salat you can get out to redo the wudu' and resume your salat from where you left off (provided you don't speak with anyone in the mean time). Is this right? And does it also apply to when you break your wudu'?
